3.3 The number of gatherings prohibited or prevented in terms of section 5 of the Regulation of Gatherings Act since 1 January 2015 to date of submission of PAIA request, and the reasons for the prohibition or prevention;
3.4 The number of gatherings authorized with conditions in terms of section 4 of the Regulation of Gatherings Act since 1 January 2015 to date of submission of PAIA request; and
3.5 The number of gatherings authorized in terms of section 4 of the Regulation of Gatherings Act since 1 January 2015 to date of submission of PAIA request.

4. Copies of Training Manuals and Training Materials (but specifically excluding those done at the national level) relating to the conduct and / or training of persons designated in terms of section 2(4) of the Regulation of Gatherings Act as a “responsible officer”.

1. All the monthly way-bills for ship slops for the period January 2010 to date;
2. AND the monthly quantity of ship slops for the period January 2010 to date;
3. AND the monthly constituents of ship slops for the period January 2010 to date;
4. AND all the monthly analysis protocols of ship slops for the period January 2010 to date;
5. AND the results of all monthly analyses of ship slops for the period January 2010 to date;
6. AND the dates that each analysis was conducted;
7. AND the details of the laboratories that conducted each analysis;
8. AND all material safety data sheets for the period January 2010 to date;
9. AND all risk assessments for the period January 2010 to date;
10. AND all atmospheric emission licences. ALTERNATIVELY, in the event that an application for such a licence has been submitted but not yet approved, the pending applications for such licences;
11. AND all waste management licences. ALTERNATIVELY and in the event that an application for such a licence has been submitted but not yet approved, the pending applications for such licences;
12. AND all water use licences. ALTERNATIVELY and in the event that an application for such a licence has been submitted but not yet approved, the pending applications for such licences;
13. AND all scheduled trade permits. ALTERNATIVELY and in the event that an application for such a licence has been submitted but not yet approved, the pending applications for such licences;

15. AND all reports compiled by the Msunduzi Municipality, the relevant Port Health Services, and other government departments in relation to compliance inspections conducted for the period January 2010 to date;
16. AND all test results, summaries, reports and associated documents relied upon by the Msunduzi Municipality, the relevant Port Health Services, and other government departments in the collation of the inspection reports referred to above;
17. AND all representations made by FFS Refiners in response to inspection reports referred to above, including but not limited to, written representations and notes detailing oral representations;
18. AND all enforcement notices and pre-notices issued by the Msunduzi Municipality, the relevant Port Health Services, and other government departments for the period January 2010 to date;
19. AND all correspondences from FSS Refiners responding to enforcement notices and pre-notices referred to above, including, but not limited to, all progress reports;
20. AND notices of fines and other penalties, both paid and unpaid, in relation to enforcement for the period January 2010 to date;
21. AND all correspondence between FFS Refiners, the Msunduzi Municipality, the relevant Port Health Services, and other government departments in relation to all inspections and enforcement action taken against FFS Refiners for the period January 2010 to date.
